Social Worker - TBI/VIST Coordinator

The Traumatic Brain Injury (TBI) Coordinator coordinates the evaluation and care of severely injured Operation Enduring Freedom, Operation Iraqi Freedom, and Operation New Dawn (OEF/OIF/OND) Veterans with highly complex health and mental health problems, providing lifetime case management with varying degrees of intervention to include:
intensive, progressive, and supportive phases based on the clinical and psychosocial presentation of each individual patient. The Coordinator is also responsible for coordinating the evaluation and care of non-OEF/OIF Veterans with TBI. The Coordinator functions as the treatment process coordinator of the TBI Interdisciplinary Team and serves as a Social Worker on this interdisciplinary team and specialty TBI Evaluation Clinic. The Visual Impairment Services Team (VIST) Coordinator serves as the case manager who has major responsibility for the coordination of services for visually impaired Veterans and their families. Duties include arranging appropriate rehabilitation services and devices in order to enhance a visually impaired Veteran's functioning level, identifying new cases of blindness, providing professional counseling, promoting an individual's psychosocial adjustment to blindness, coordinating care to meet Veteran's resource needs, meeting specific objectives established by the VIST, arranging VIST reviews, and conducting outreach and educational programs relating to VIST and blindness. The incumbent is responsible for completing the following duties/responsibilities As a member of the TBI and VIST Interdisciplinary team, participates fully in developing, planning, implementing and evaluating the interdisciplinary treatment plan, including provision of care management services. Addresses all assigned TBI and VIST consults within the Cheyenne VAHCS, to include CBOCs, MSOCs, and mobile clinics. Serves as a consultant for all staff regarding TBI and VIST clinical reminder screenings. Manages the daily operation of the TBI and VIST programs, develops policies and procedures for program operation, and prepares reports and statistics for facility, VISN, and national use. The incumbent will provide a full range of social work services within commonly accepted standards of independent social work practice, to include triage, advocacy, individual, family, and group counseling, discharge planning, case management, crisis intervention, and other services as appropriate. The incumbent will participate in care planning with the interdisciplinary team, including whenever possible the Veteran and Veteran's caregivers in the identification of treatment goals and preferred outcomes. The incumbent will provide consultation/education to Veterans and caregivers on VA and community resources, benefits, and support services specific to the Veteran's or caregiver's needs. The incumbent will act as a consultant to other team/staff members, providing excellent customer service, advocacy, and education on relevant topics to internal and external customers. The incumbent will act as liaison between VA and community resources, to include marketing and public relation duties as needed, and fostering knowledge and collaboration between VA and community partners. The incumbent will provide mentorship and supervision to social work students, or less experienced social workers, as appropriate. The incumbent will actively participate in committee assignments, as the Supervisor deems necessary and advantageous to the organization. The incumbent will participate in quality improvement projects and practices, to include peer chart reviews, data collection, and monitoring customer satisfaction, in compliance with JCAHO, CMS, State, VHA and CHYVAMC policies and procedures. The incumbent will attend staff meetings and any other mandatory meetings regularly, notifying the supervisor when an excused absence is necessary. The incumbent will remain current and in good standing in their licensure, privileging or scope of practice, and educational requirements. The incumbent will follow National Social Work Ethics and Rules of Behavior, advancing the standing and reputation of the profession at all times. The incumbent will perform other related duties, as assigned by the supervisor. The incumbent reports to the Chief, Social Work Service, or designee. The incumbent is responsible to the Chief of Social Work or designee but practices independently, exercising initiative and professional judgment in day-to day activities and in making decisions about services and treatment provided to the Veteran. The incumbent receives supervision and guidance from the Chief, Social Work Service or designee for clinical, administrative, and/or programmatic issues. Work Schedule:

Experience:
Individuals assigned as social worker program coordinator or manager must be licensed or certified at the advanced practice level. Their experience must demonstrate possession of advanced practice skills and judgment, demonstrating progressively more professional competency. They may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience. Experience in a medical social work field, to include polytrauma/TBI and sensory impairment. Experience in care coordination with interdisciplinary teams.
